MSK Graduate Staff Application for Full-Time Trainees
Application Process & Instructions
In preparation for your upcoming rotation at Memorial Sloan Kettering, please review the instructions below regarding the appointment process.
Step 1: Complete Application
Complete Application
- You will be asked to create a profile, which will include your Social Security Number (SSN) and Date of Birth.
- You will be required to complete all sections of the application –however you do not need to complete this in one session.
- Be sure to use a personal email address to which you will always have access.
Step 2: Upload Documentation
Please upload the following documents within the application.
- Curriculum Vitae (Resume) - Please ensure CV is current and in mm/yyyy format
- Medical School Diploma (and translation if not in English)
- Copy of a government-issued Photo ID
- ECFMG Certificate (if applicable)
- Signed Clinical Privileges (template provided by your MSK Program Coordinator)
- Certificates of completion for Infection Control Training, Child Abuse Recognition and Reporting Training, and NY State Mandatory Prescriber Training (if applicable)
- Visa Documentation (if applicable)
Step 3: Document Certifications

In the Certifications section, please make sure to include your NPI number, ECFMG number (graduates of international medical schools only), and other applicable identifiers as indicated in the instructions.
Step 4: Identify Education Programs
Identify Education Programs
In the Education and Activity record, list all education programs in which you were granted a degree, and any clinical training programs in which you have been or are currently enrolled.

Once you submit your application it will be forwarded to your MSK Program Coordinator for review and presentation to Graduate Medical Education (GME) and the Medical Board for appointment.
